Recruitment, Retention and Training Incentives (RRETI) Program

What you will need to complete this form:

• A current copy of registration with the appropriate regulatory body in B.C. (e.g., B.C. College of Nurses & Midwives, B.C. Society of Laboratory Science, etc.)
• If applying as a ‘Newly Recruited Health Care Professional’, and prior to your official start date, a signed letter of offer from the health employer.
• If applying under the ‘Residents Completing Health Care Training’ program, a copy of your official post-secondary transcripts to be sent directly by your post-secondary institution to the NRRM, c/o Corporate Manager, Northern Rockies Regional Municipality, 5319 - 50 Avenue South/Bag Service 399, Fort Nelson BC, V0C 1R0 or jmaciver@northernrockies.ca via email

CONSENT TO USE/DISCLOSURE OF PERSONAL INFORMATION:
I consent to the use and disclosure by the Northern Rockies Regional Municipality (NRRM) of my personal information for the purposes of:

• Release of my personal information with my health employer, as required, for the purpose of managing the Reward of Service component of this Program; and
• Using any photograph(s) that I have provided to the NRRM in connection with my application for the RRETI program, and any other photographs or other images of myself (including any likeness or image in still, digital, videotape or other format, and whether I appear alone or together with one of more other persons), that the NRRM (or others on behalf of NRRM may have taken, made or recorded in connection with my participation in the RRETI program.

I understand that this information will be used for the purposes described above, and that any other use will be subject to further consent.
